Reach out to us Interaction Effect An exaggerated hypotensive response to the first dose of the alpha blocker Interaction Summary In patients currently on beta blockers, the first dose response (marked initial drop in blood pressure, particularly on standing) to an alpha-1 adrenergic blocking drug may be exaggerated. These effects may be less marked with alpha blockers other than prazosin , but caution should still be exercised with all alpha blockers. The lowest possible initial dose should be used and the first dose should preferably be given at bedtime. The addition of beta blocking drugs to ongoing alpha blocker therapy is much less likely to lead to a significant fall in blood pressure . Severity Moderate Onset Rapid Evidence Probable How To Manage Interaction When concurrent therapy with an alpha blocker and a beta blocker is required, initiate the alpha blocker with a smaller than usual dose, preferably at bedtime. Monitor the patient closely for hypotension. Mechanism Of Interaction Suppression of beta-mediated compensatory increases in heart rate Literature Reports A) Administration of a single dose of atenolol 100 mg with a single dose of immediate-release alfuzosin 2.5 mg in 8 healthy young male subjects increased alfuzosin Cmax and AUC values by 28% and 21%, respectively. Alfuzosin increased atenolol Cmax and AUC values by 26% and 14%, respectively. The combination of alfuzosin with atenolol caused significant reductions in mean blood pressure and in mean heart rate in this study . Metoprolol Overview Metoprolol is used alone or in combination with other medications to treat high blood pressure. It also is used to treat chronic (long-term) angina (chest pain). Metoprolol is also used to improve survival after a heart attack. Metoprolol also is used in combination with other medications to treat heart failure. Metoprolol is in a class of medications called beta blockers. It works by relaxing blood vessels and slowing heart rate to improve blood flow and decrease blood pressure. High blood pressure is a common condition and when not treated, can cause damage to the brain, heart, blood vessels, kidneys and other parts of the body. Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. In addition to taking medication, making lifestyle changes will also help
